Document new features if Eshell

* doc/misc/eshell.texi (Input/Output): Document the new
'#<bufname>' syntax.
(Input/Output): Document 'eshell-destroy-buffer-when-process-dies'.
Disable "Key Index" generation, as there are no @kindex entries in
this manual.
