Have 'vc-prepare-patch' handle prefix arguments.

* lisp/vc/vc.el (vc-prepare-patch-prompt-revisions): Extract common
function and handle prefix arguments.
(vc-prepare-patch): Pull logic out to
'vc-prepare-patch-prompt-revisions'.
